Please follow these easy directions:

1.   Open the form, save the form to your computer. Doing this will give you the option to see the typewriter tool on the top of the form.

2.   Put your cursor (mouse) on the paper where you want to type and click that area, then start typing. You must select the typewriter for each area you need to fill in.

3.   Save copy of the form to your computer.

4.   Print and return the completed form to the school secretary.
